GAPC EDUCATIONAL PROGRAM

Presented by: Shannon Lozano - Hospice Care Consultant, AccentCare Hospice & Palliative Care

This presentation explores how to approach hospice conversations with patients, families, and healthcare providers in a compassionate, informed way. Attendees will gain a deeper understanding of hospice care as a supportive, dignity-centered approach to end-of-life care, including when and why hospice is appropriate. The session addresses common misconceptions, clarifies eligibility and services, and highlights the importance of aligning care with a patient’s goals, comfort, and quality of life. Emphasis will also be placed on the interdisciplinary nature of hospice care and the emotional and spiritual support available to patients and their loved ones.

    Shannon Lozano is a dedicated Hospice Care Consultant with 10 years’ hospice experience. She is committed to helping patients and families understand their options during some of life’s most difficult moments. She works closely with hospitals and healthcare providers to ensure patients facing serious illness receive compassionate, timely hospice support.

    Shannon believes hospice is about dignity, comfort, and honoring what matters most. Through education, collaboration, and advocacy, she helps families feel confident in their care decisions and supports healthcare teams in delivering patient-centered end of life care.
